The Kum Do School listed above is an
authorized school of the World Kum Association. The above listed Kum Do School
teaches martial arts that are in conjunction with the rules and guidelines of
the World Kum Do Association.
My membership into the World Kum Do
Association is contingent upon signing this membership agreement with final
approval by the World Kum Do Association.
I, ______________________have fully read
and agree to abide by and conform to the rules and guidelines of the World Kum
Do Association in the following agreement:
As a martial art student enrolled in a
course of Kum Do study at the above listed school, I agree to maintain a regular
attendance in the martial art study. I agree to keep the curriculum private and
not to communicate, disclose, or instructed to any third party other than
another World Kum Do Association instructor or member.
Furthermore, if I discontinue my martial
art study at the above listed Kum Do school, or any other affiliated school, I
agree not to use any of the curriculum concepts or the name of the World Kum Do
Association in the future.
This agreement is governed by the laws of the state of Iowa. In addition, by signing this agreement, I consent to personal jurisdiction, service and venue in any Federal or State Court within the state of Iowa.
